No Login Required

When using the External Forms feature for PIT data collection, the form enforces the collection of required HMIS elements, but can be customized to add additional questions using the standard form editing tools.  

Volunteer Friendly

The tool offers customizable guidance for volunteers to help them successfully complete each survey.

Multi-Modal

The PIT tool understands that not every PIT interaction is verbal, and provides different data collection questions for in-person and observation only interactions.

Spot On Reporting

The Open Path PIT Tool includes the ability to collect geographic coordinates via the device in-use. 

The volunteer can click the Get Current Location button when they are meeting an individual experiencing homelessness, and a pin will be dropped on the map to indicate where the interaction happened. 

These pins can be seen within the Open Path Warehouse and used for reporting and data analysis in the OP Analytics tool.

Simplified Data Collection

Whether you are interacting with the individual or family, or simply collecting observation-based data, the PIT Tool streamlines data collection.   Questions are focused on required HMIS PIT data collection and the tool offers guidance for volunteers to help them successfully complete each survey.

Household Aware

The Open Path PIT Tool is aware that not every PIT interaction is with an individual and provides streamlined surveys for each household. 

If you choose to collect the survey for a family, you are prompted to collect responses for each household member, and when imported to HMIS, the household members will remain connected.

A survey form asks if the user is an Individual or a Family, with Family selected for the pit count. A note reminds users to complete a survey for each family member. Below, there is a blue Submit Form button.
A confirmation message reads, Success. Your pit count response has been recorded. Below are two buttons: one labeled Add Another Household Member and another labeled Done.

Streamlined HMIS Enrollments

Since the Open Path PIT Tool is tied directly to a full-featured HMIS, data is fed directly into a street outreach project. 

Each submission can be reviewed and turned into an enrollment individually, or submissions can be bulk converted.  

The Open Path HMIS treats External Form submissions as untrusted data and sanitizes input before import and provides protections and tools for dealing with spam submissions. Before an enrollment is ever added to the project, an authenticated user must review and approve the incoming data.
